I have been seeing Dr. Rikhter for more than a decade. Dr. Rikhter is very professional and thoughtful doctor. He always hears my concerns, is always trying to address each point and detail and never he is never dismissive. He always makes you feel that you as a patient is important. As a bonus, you can do blood work straight in the office. All staff is very pleasant and accommodating. I highly recommend Dr. Rikhter as a family doctor.

There has been extensive data over the past decades indicating endometriosis may be linked to select co-morbid conditions in some individuals with the disease as well, including but not limited to a low/modest association between certain pigmentary traits and melanoma; pain syndromes (interstitial cystitis/painful bladder syndrome, irritable bowel syndrome/inflammatory bowel disease, chronic headaches, chronic low back pain, vulvodynia, fibromyalgia, temporomandibular joint disease, chronic fatigue syndrome, etc.) as well as mood conditions (defined as depression and anxiety) and asthma; select infections and endocrine disorders; headaches and migraines; thyroid disease and others. Similarities in the clinical and epidemiological features of the associated disorders may be at the root of their co-morbidity, and further investigation is needed. Recently, an association between laparoscopically proven endometriosis and future stroke has been discovered (though it is important to note that mediating effects i.e., hysterectomy, oophorectomy need further investigation).

PRP, or Platelet Rich Plasma is a product derived from the blood of the patient undergoing surgery. Plasma is the blood product that is left over after the red blood cells have been removed. This is further concentrated to form platelet poor and platelet rich plasma. We take the platelet rich plasma, and spray or inject it onto the raw surfaces that are present after excision of endometriosis and adhesions. A study in 2007 demonstrated reduced pain following laparoscopic surgery with the use of PRP. Initial use of it in our practice demonstrated this as well. One of the additional benefits that we saw was a reduction of adhesion formation in the patients that we operated on. We saw a reduction of surgeries needed for adhesion induced pain, and also, when we re-operated on patients for any reason, we saw that those that had PRP used, had a reduction in adhesion formation compared to those who had not received it. This makes sense because PRP has been used years in several other areas of medicine to help with healing such as orthopedics, plastic surgery, ophthalmology, and orthodontics are just a few. The mechanism of action likely has to do with the presence of growth factors within the PRP and this promotes healing, and through faster healing, we have less chronic inflammation and adhesion formation. We have used it in close to 2,000 patients since 2013 with excellent results.

CEC surgeries range from LAPEX of highly complex, extrapelvic (thoracic, etc.), deeply fibrotic endometriosis including urogenital/colorectal involvement and retroperitoneal dissection to myomectomy, Laparoscopic hysterectomy, adhesiolysis, tubo-ovarian procedures and far beyond. Endometriosis & Malignancy: in the past decade, a potential cancer risk in association with the disease has emerged. Increased risk factors include early-stage/low-grade disease and a specific histology i.e., endometrioid or clear cell carcinoma, but much is still unknown about the relationship. To that end, robust research is underway to evaluate the clinicopathologic characteristics of endometriosis-associated ovarian cancer (EAOC) in comparison with non-EAOC as well as other potential links. It is also true the potential exists for endometriosis to malignantly transform, but this again is a poorly understood and uncommon phenomenon. Nonetheless, much research remains to be done to better illustrate and understand the tenuous link between endometriosis, ovarian cancer and other potential malignancies.
Endoscopy
Our Center has been using PRP now for years, across several hundreds of patients to date. Based on our follow-up and observational data in combination with practical experience, we have found PRP to be a viable option as an adjunct to our Laparoscopic Excision and gynecologic endoscopy procedure(s). While we have only needed to re-operate on less than ten of those PRP patients, the re-operative results imply there is less adhesion formation - hence less adhesion-related pain, as well as less fibrosis (which can cause pain as well). Again, PRP is not new to our discipline - for example, even 15 years ago, a phase I/II trial of autologous platelet tissue graft in gynecologic surgery designed to evaluate toxicity and efficacy on decreasing pain was conducted of 55 consecutive patients undergoing gynecologic surgery, matched with 55 patients from the previous 6 months. Results from this study revealed median pain on the day of surgery (2.7 - mild) in the autologous platelet tissue graft group vs. 6.7 (severe) in the control group. Likewise, pain on postoperative day 1 was 2.1 (mild) in the autologous platelet tissue graft group vs. 5.5 (moderate) in the control group. Median of morphine per hospital stay for the autologous platelet tissue graft group was 17 mg (range 1-98 mg) vs. 26 mg (range 3-90 mg) in the control group, which was statistically significant at p=.02. Importantly, there were no adverse effects associated with autologous platelet tissue graft, and patient pain was significantly reduced [

MS: Hydrate hydrate hydrate both pre and post op - helps with dehydration, flushes the anesthesia, and helps get the bladder to wake up and not get a uti. Don't do anything strenuous but don't lay in bed either. Take short naps and then get up every hour and walk...even if it's just in circles around your house. The walking promotes healing...and gets rid of the gas pain that can be worse than surgical pain. Walk when they tell you to at the hospital. Don't lie flat...it makes residual gas pain flare up. The tummy binder is your best friend but give yourself a break from it off and don't make it too tight. Don't hesitate to call the doc after surgery if you sense something isn't right...its probably true. Don't be shocked if you still have pain post op. Prepare to have Lots of things to occupy your time...your brain will feel clear and ready long before your body.

Upon graduating from Medical School at Universidad Iberoamericana, Dr Eugenio-Colón completed a Fellowship at Southern Illinois University School of Medicine and Residency at Lincoln Medical and Mental Health Center. An avid researcher whose interests include pelvic pain, evidence-based and multispecialty treatment of endometriosis, and development of algorithms for endometriosis treatment and follow-up, he has countless presentations, lectures and publications to his credit. One of the concerns about treating patients with ovarian endometriosis/endometriomas (endometriosis or 'chocolate' cysts on the ovaries) is that ovarian reserve - the amount of healthy eggs you have - may be affected by excising the endometriomas. This is a controversial and constantly evolving issue in endometriosis research and infertility. First, those with other types of cysts have better ovarian reserve before and after surgery compared to individuals with endometriomas. This suggests that endometriomas may affect ovarian reserve just by their presence. However, if an endometrioma is not removed, the endometrioma may grow in an unpredictable fashion, resulting in a potentially larger loss of ovarian reserve. For these reasons, several professional organizations such as the European Society for Human Reproduction and Endocrinology and the Society of Gynecologists of Canada recommend removal of endometriomas when they are larger than 3 or 4 cm. The eggs next to the endometrioma are of poorer quality, and will allow for better quality eggs to remain and if IVF is needed, there will not be contamination of the eggs that are retrieved which can happen when the fertility specialists removes the eggs for fertilization.

MRI should be expected to be excellent in recognizing uterine masses like fibroids, cysts, and adenomyomas if they reach 5 mm or greater in size. We expect that it will also add to the ability to differentiate among any of the above. MRI may be able to lead us to expect adenomyosis if the myometrial thickness is increased or the consistency of the myometrium is changed. Unfortunately, this type of information will probably remain quite nonspecific, and as noted above, imaging is heavily contingent on the interpreter of the images. I am not hopeful that we will soon be able to rely on it to diagnose the isolated, scattered areas of glands lost among the muscle cells because of their small size. Much work is ongoing to get more information as to the diagnostic accuracy of this technique.

Many patients have seen gastroenterologists (GI doctors) for assessment of possible GI causes of their pain, often more than one doctor, having extensive testing including CT scans, endoscopies, colonoscopies, GI motility studies, MRI enterography, etc., often costing thousands of dollars. If the initial doctor would focus on the underlying pelvic pain, they would understand that

***Severely immunocompromised patients encompass a population including cancer patients or others whose immune system may take longer to recover from COVID-19 due to their disease state or cancer treatment. Examples include but are not limited to blood-related cancer, Myelodysplastic Syndrome, receiving chemotherapy for a solid bone tumor, a genetic mutation or disease that causes loss of immune function that is poorly managed (such as HIV), previous organ transplant and taking anti-rejection medications, previous bone marrow transplant and taking immunosuppressive medications, and those with a primary immunodeficiency (such as Wiskott-Aldrich Syndrome, DiGeorge Syndrome, Severe Combined Immunodeficiency Disease (SCID), or Chronic Granulomatous Disease)

Sciatica
Physical examination may reveal various neurological deficits involving the sciatic nerve rootlets. There may be localized tenderness over the sciatic notch, but this is not always found. Pelvic examination may also even be normal. Imaging can aid in diagnosis, though ultimately a surgical diagnosis is indicated. Early diagnosis and treatment are critical to minimize the damage. While sacral radiculopathies (pudendal, gluteal pain), vascular entrapment or sciatic neuralgia may be at the root of symptoms for some individuals, in patients with sciatica of unknown genesis and/or suspicion of pathology such as endometriosis, laparoscopic exploration of the sacral plexus and/or sciatic nerve is advisable.
Minimally Invasive Surgery
Most of all, this was my first surgery where I didn't experience any complications. For example, unknown or wound infections, bladder issues & incompetent hospital staff. Dr. Sinervo & staff took very good care of me before, "DURING" & after surgery. I emphasize "DURING" since his careful, skillful process took away scar tissue with very little to no bleeding & new tissue is not expected. One can imagine how meticulous a surgeon must be to do a minimally invasive surgery with no damage to other organs. I highly recommend Dr. Sinervo! For any scar tissue... HE'S YOUR MAN!
Orthopedic Surgery
Vascular Surgery
Coseal and Tisseel are products that have been advocated for use in prevention of adhesions. These are products that indicated for use in vascular surgery to reinforce a graft to help with hemostasis. It should not be used for adhesion prevention. Similarly, Tisseel has been used for hemostasis when traditional options have failed and for use to reinforce the site of a bowel resection. There are some small studies that have suggested benefit but these are not approved by the FDA for adhesion prevention. It can be used as an off-label use, but my experience has shown that when used in the pelvis, these products result in more adhesions. Similarly, Arista, a product intended for use for hemostasis has also been suggested for off label use for adhesion prevention. It is made up of microporous polysaccharide spheres (Arista) that form a polysaccharide powder made from potato starch. It absorbs water, concentrating platelets and other proteins to accelerate clot formation. In the patients that we have seen it used in, we have also seen increased adhesion formation above what we would expect to be normal after the described surgery, so we do not recommend its use, either.
Hysterectomy
Imaging diagnostics may show features of adenomyosis with relatively high sensitivity. However, interpretation of findings is operator-dependent. Treatments remain a major challenge, with hysterectomy (removal of the uterus) still the only definitive cure. It can be very difficult to remove adenomyosis partially, and recurrence after adenomectomy can be quite high.

Today, we know PRP has the potential not only to improve hemostasis, but also to contribute greatly to the biological effect on wound healing. Several advancements in this innovative area of therapy (CPT 0232T) have been brought forth over the past decade, and the body of literature continues to grow at rapid pace with the procedure gaining traction. The disciplines of (including but not limited to) plastic surgery, dentistry, musculoskeletal, otolaryngology, spinal, and gynecology all use PRP therapy to deliver growth factors to optimize healing in their patients. In the United States, even early application was estimated to be used in Obstetrics & Gynecology specifically in more than 100,000 cases annually [
Endometrial Ablation
Pain management and/or conservative treatments can be tried (IUD or other suppressive hormones, for example). If the adenomyosis can be visually recognized, as with an adenomyoma, local excision or cauterization during laparoscopy can be effective. We have removed many localized areas of adenomyosis with good symptom relief. Unfortunately, however, most of the time the disease is scattered throughout the uterine muscle. Attempts to control the symptoms of deep adenomyosis with endometrial ablation (a different procedure from ablation of endometriosis) have not been successful.
